Fast Application

By checking the Fast Application checkbox on the Vettir Registration page, I agree to allow Vettir to automatically fill (auto-fill) my application with the relevant details required to complete my application. Vettir may use my first and last name to auto-fill my signature and a portion of my first and last name to auto-fill my initials on the Application.

I allow Vettir to automatically skip through steps of the Application that are fully complete. I understand that the Fast Application service may contain unexpected or unanticipated behaviors and I allow these behaviours to take place as needed to complete my application.

I allow the Fast Application to make inferences about relevant application information from information collected on the Registration page, and other pages on Vettir.

 

Disclosures

I understand I have the right to enroll in the state LifeLine service using non-electronic methods. I also have the right to withdraw this consent at any time prior to activation of my service. I may request a paper copy of my contract and associated fees by calling 611 from my wireless handset.

state LifeLine provides discounted phone service. I understand I am entitled to a voice-grade connection. I understand I have the option to receive instructions for the Application Form in standard or large print.

This service is a prepaid service and you must personally activate it by calling 611 from your Vettir handset. To keep your account active, you must use your state LifeLine service at least once during any 30 day period. At 30 days of non-use, Vettir will provide notice to the subscriber that failure to use the state LifeLine Services within the subsequent 15-day notice period will result in de-enrollment. Subscribers can use the Services by: (1) completing an outbound call or the usage of data; (2) purchasing minutes or data from the Company to add to the subscriber's service plan; (3) answering an incoming call from a party other than Vettir; (4) responding to direct contact from Vettir and confirming that the subscriber wants to continue receiving the service; or (5) sending a text message. If the subscriber does not respond to the notice, the subscriber will be de-enrolled (allowing only calls to 911). This usage policy applies only to customers who do not have a regular billing relationship with the Company. By answering yes, I hereby certify under penalty of perjury that I have read and understood the disclosures listed above regarding activation and usage requirements.

I acknowledge that I am providing the information I have included in this application to Vettir and further authorize Vettir to receive and use my information for enrollment verification and waste, fraud and abuse mitigation purposes. Additionally, I authorize Vettir to receive and use my historic LifeLine enrollment information for enrollment verification and waste, fraud and abuse mitigation purposes.

I hereby authorize Vettir to access any records required to verify my statements on this form and to confirm my disclosures for the state LifeLine program. I also authorize Vettir and the state LifeLine Administrator to release any records required for the administration of the state LifeLine program (name, telephone number, address, date of birth, last 4 digits of SSN, amount of support being sought, means of qualification for support, and dates of service initiation and termination), including to the Universal Service Administrative Company or the state Lifeline Administrator, to be used in a state LifeLine disclosures database and to ensure the proper administration of the state LifeLine Program. Failure to consent will result in denial of service.

I hereby certify under penalty of perjury that I meet the income-based or program-based disclosures criteria for receiving state LifeLine service and have provided documentation of disclosures if required.

I hereby certify under penalty of perjury that I will notify Vettir within 30 days if for any reason I no longer satisfy the criteria for receiving state LifeLine including, as relevant, if I no longer meet the income-based or program-based disclosures criteria, I begin receiving more than one state LifeLine benefit, or another member of my household is receiving a state LifeLine benefit. I understand that I may be subject to penalties if I fail to follow this requirement.

I hereby certify under penalty of perjury that I am not listed as a dependent on another person's tax return (unless over the age of 60). I hereby certify under penalty of perjury that the residential address listed is my primary residence, not a second home or business.

I hereby certify under penalty of perjury that if I move to a new address, I will provide that new address to Vettir within 30 days.

I hereby certify under penalty of perjury that I acknowledge that providing false or fraudulent information to receive state LifeLine benefits is punishable by law.

I hereby certify under penalty of perjury that I acknowledge that I may be required to re-certify my continued disclosures for state LifeLine at any time, and my failure to re-certify as to my continued disclosures within 30 days will result in de-enrollment and the termination of my state LifeLine benefits.

I hereby authorize Vettir to send text messages to my Vettir provided wireless number about my state LifeLine benefit. Text messages sent by Vettir will not decrement my available wireless minutes or texts. Standard voice, data and text rates will apply to all messages to and from anyone other than the Company.

If Vettir finds that I am already receiving a state LifeLine discount benefit from another provider, I agree that I want to transfer my state LifeLine discount benefit from that state LifeLine provider to Vettir. I understand that once the transfer is complete, I will lose my state LifeLine Program benefit with any other state LifeLine provider from which I am currently receiving a state Lifeline discount. Vettir has explained to me and I understand that I may not have multiple state LifeLine Program benefits with the same or different providers.

I hereby expressly authorize and direct Vettir to allow a representative of Vettir to COMPLETE, SIGN on my BEHALF, AND SUBMIT MY state LIFELINE APPLICATION, along with my supporting documentation to the state LifeLine Administrator to complete the state LifeLine signup process.

I understand I am entitled to a voice-grade connection and that if, at any time, I am unable to receive a voice-grade connection and notify Vettir, the company will: (1) promptly restore the voice-grade connection, or if not possible, (2) provide telephone service using different technology if offered by Vettir and I agree; or (3) allow me to discontinue service within 14 days of service activation without incurring early termination fees. Vettir does not require participants to enter into a service contract, so participants are free to discontinue service at any time without penalty.

I hereby certify that I have reviewed all the information contained in this application and have been given the opportunity to correct or cancel this order and re-submit a new one.

I hereby certify under penalty of perjury that the information contained in this certification form is true and correct to the best of my knowledge.

I hereby certify that, I live alone or live with other adults who do not receive Lifeline or the Emergency Broadband Benefit

I understand I am only allowed to get one Lifeline benefit and one Emergency Broadband Benefit ( if applicable) per household, not per person.

I understand that this limit is an FCC rule, and lying about my household on this government form can make me lose my Lifeline benefit or my Emergency Broadband Benefit and is against the law.
